OUTSIDE PAID SERVICES
Many AYSO Regions and Areas hire outside contracted services for landscaping and other field services, coach and player training support, soccer camps, accounting, and food services, among others. AYSO has created a one-stop, six-step procedure to assist Regions quickly complete the process under AYSO National Policy Statement 2.5.
Paid Services Agreement & Questionnaire
To begin the process, download and complete the appropriate Paid Services Agreement Form:

Paid Services Approval Packet
Send the following documents to riskmanagement@ayso.org:
- Paid Services Agreement
- Paid Services Questionnaire
- Provider's Certificate of Liability Insurance naming AYSO
Download Sample Certificate of Liability Insurance
- Provider's Business License obtained from city or county
- Completed W-9 from Provider
- For California, also include the Independent Contractor Compliance Form
