Completing 1099 Template

AYSO has created a template to streamline the submission of 1099 information. Timely and accurate reporting helps to avoid costly penalties and interest imposed by the IRS.

  • The template can be opened in Excel or, if you don't have access to Excel, can be opened in Google Docs.
  • Even if you completed a 1099 Template in a previous year, please download a new version from the above link
  • Each independent contractor/vendor should be listed on its own line

Fields and Explanations

Many of the columns are self-explanatory, however you can find more information on select columns if there is any confusion.

  • Tax Id # - Tax ID could be a social security number or a TIN provided by the IRS
  • Name (line one) - Name is line 1 of the W-9 form. Often this will be the individual's name or the name of their business
  • Name (line two) - optional - This optional field can be used for additional company or individual names if applicable
  • Address1 - This is the street address for the Individual or business for us to mail the 1099 to
  • State - Please use the standard 2 letter State abbreviation (i.e. CA for California)
  • Type of Service Provided - This is a drop down field for you to select the type of service provided by the vendor. If you don't see the service provided listed, please select "Other/ Not Listed" and indicate the service in the notes column
  • Notes - this column is necessary unless you need to clarify the type of service provided
  • Email - whenever possible, please supply the vendor's email. Often, vendors will email their completed W-9 to you, giving you their preferred email
  • Phone Number - Whenever possible, please supply a phone number for the vendor.
  • Total Amount Paid to Vendor 1/1/2024-12/31/2024 - Only include amount paid for the calendar year of 1/1/2024 - 12/31/2024.
  • W-9 Collected by Region? - This yes/no selection indicates whether or not you have collected the W-9 from the vendor and are sending it with your submission.
