Executive Member Appointment Forms

Every three (3) years (required every 3 years. (AYSO National Bylaw 7.01-7.03)) or when there is a vacancy in an Executive Member (EM) position a selection process or election should be held to fill the position. For guidance on this process visit Executive Member Selection Protocol. Once the EM-elect is chosen by the respective board, the appropriate forms should be completed during an Onboarding Orientation with the Candidate and the Area/Section Director.  The orientation should include the following: The appropriate EM Appointment Form and Conflict of Interest Statement should be reviewed and filled out together. These two forms are submitted to the AYSO Office for the Candidate's approval by the National Board of Directors. The EM Appointment Checklist should also be reviewed and filled out together.  This form becomes a tool for the AD and Area Staff and the SD and Section Staff to help the EM fill any gaps in volunteers and training and to be able to understand the status of the Program.  Finally, the EM Job Checklist should be reviewed to help the RC understand their role and to ensure they have the tools necessary to fulfill their role.  This orientation should take approximately 3 hours for a new EM or 1-1 1/2 hours for a returning RC.  (Orientation can be done in a single session or multiple sessions).
