Required: Known Medical Conditions

This question has been a standard part of AYSO registration for many years. It ensures that coaches, team parents, and other program staff are aware of any physical conditions that could affect a player's safety or participation during practices and games.

Why This Question Matters

AYSO's first obligation is player safety. Coaches and team parents are often the first responders when a player experiences a medical episode on the field. Without this information, staff cannot take appropriate precautions or respond effectively in an emergency. Examples of conditions families may disclose include asthma, severe allergies (including bee sting or food allergies), diabetes, epilepsy, and cardiac conditions.

This question also complements the Emergency Treatment Authorization included in the Player and Parent Agreement & Liability Release, which authorizes AYSO staff to consent to emergency medical treatment on behalf of the participant.

Recommended: Volunteer Background Check Donation

This question is new for MY26, introduced in response to a change in how AYSO processes volunteer background checks. With the transition to JDP as AYSO's new background check provider, volunteers can no longer pay for their own screening directly through the provider's portal — a process that previously allowed costs to be recovered at the individual level. This question will allow volunteers and those not volunteering to contribute to the cost of safeguarding players.

Why This Question Matters

AYSO's transition to JDP as its background check provider means that volunteers can no longer pay for their own screenings directly if the Regions maintain a credit card on file with JDP and are billed as checks are run. This shifts the total financial burden to the Region.

Including a voluntary $25 donation prompt during player registration gives families an easy, frictionless way to contribute to the cost of keeping the program safe. Even partial uptake across a region's registration base can meaningfully offset screening expenses, helping ensure that background check costs do not strain Regional budgets.

The inclusion of "AYSO's player-focused mission" in the question language is deliberate — it ensures contributions are classified as unrestricted donations, giving regions the flexibility to apply funds where they are needed most rather than being obligated to the single designated expense of volunteer background checking.
