Referee Reports (formerly known as a “Misconduct Report”) are to be completed by the Referee to capture all pertinent facts and contact information whenever there is misconduct by AYSO team officials or players that occurs before, during, or after the match including any send-offs of players or team officials; or for any instance of potential Referee Abuse by players or team officials that occur. This report includes information on any disciplinary action taken by the Referee. Each Referee Report must be signed by the Referee of the match and submitted to the appropriate Referee Administrator within 24 hours of the incident.

Incidents involving potential Referee Abuse by spectators should be reported on an AYSO Incident Report Form and also submitted to the appropriate Referee Administrator.

It is the governing body’s responsibility to review the Referee’s match report and to take whatever additional action they judge appropriate.

Note: Cautions issued by the Referee are recorded on the back of the match/lineup cards. A separate Referee Report is typically not required.
