Authorization to Release Player

Once a player has been delivered to an AYSO practice, game or event, the coach remains responsible until a duly designated adult has taken charge of each child after a practice, game or other AYSO activity.

AYSO does not encourage having children walk or ride a bicycle home or to a friend or relative’s house after a practice, game or other AYSO activity. However, we recognize that it may be necessary in some cases.

Neither AYSO nor the coach can be responsible for a child’s safety if they do walk or bike home. A parent or guardian assumes full responsibility for the safety of the child once the child is released from a practice, game or AYSO activity. Please note that this also applies in the case of players who are permitted to drive themselves to and from any AYSO activity.

No child may be left unsupervised after any AYSO activity. Parents who are unreasonably late or consistently tardy may be reported to the Regional Child and Volunteer Protection Advocate (CVPA) for action. Each coach may establish a standing policy for where children may be picked up by late parents.

If your child will be carpooling with another adult, or walking, riding a bicycle or driving themselves home from a practice, game or other AYSO activity, please complete the form below authorizing your coach to release the player.
