A Certificate of Insurance or proof of liability coverage, is usually required by property owners such as school districts, cities and their recreation departments and private businesses before they grant a Use Permit to any organization requesting permission to use their fields or facilities.   Because AYSO&#039;s insurance policies renew on July 1, of each year.  New Certificates of Insurance will be issued each year on July 1.  Requests can be made prior to the renewal period, but Certificates will only be emailed to the Holder as of July 1st each year. &lt;br /&gt;

There is no coverage available for the following activities as outlined in AYSO National Policy Statement 2.16 Excluded Activities. Regions are advised that the following activities are excluded from AYSO’s insurance coverage and must not be included as AYSO sponsored activities: &lt;br /&gt;
&lt;br /&gt;
*Inflatable amusement devices which participants can climb or play on.&lt;br /&gt;
* Fireworks or the sale of fireworks.&lt;br /&gt;
*Amusement or carnival rides.&lt;br /&gt;
*Tractor pulls, hay rides or riding on parade floats.&lt;br /&gt;
*Rock climbing walls.&lt;br /&gt;
*Bungee jumping or bungee type rides.&lt;br /&gt;
* Trampolines.&lt;br /&gt;
*Other potentially high risk, non-soccer related activities.&lt;br /&gt;
&lt;br /&gt;
Additionally, AYSO insurance does not cover adults, including registered volunteers, playing in a game of soccer. This exclusion includes coach versus referee goodwill games, parent scrimmages and especially parent versus player scrimmages. Because of the risk of serious injury, under no circumstances should adults engage in scrimmages or games against minors. Adults registered in the AYSO Adult Soccer program will be covered by the Adult Soccer program insurance for official AYSO Adult Soccer activities.&lt;br /&gt;

&lt;div&gt;AYSO purchases Soccer Accident Insurance (“SAI”), which pays excess medical costs up to $50,000 maximum per accident to an insured person for accidental bodily injuries incurred as a direct result of participation in a covered activity subject to the policy terms, conditions and exclusions. Eligibility for benefits is 52 weeks from the date of a covered accident. See the Maximum Benefits Payable listed below for a more detailed explanation of the plan limits. Accident Medical Expense Benefits are only payable:&lt;br /&gt;
&lt;br /&gt;
* For usual and customary charges incurred after the deductible has been met; &lt;br /&gt;
* For those medically necessary covered expenses that the covered person receives;&lt;br /&gt;
* If first medical or dental expense must be incurred within 60 days of covered accident;&lt;br /&gt;
* Proof of loss is filed within 90 days;&lt;br /&gt;
* Each claim is subject to a $500 Deductible; and 20% member Coinsurance; &lt;br /&gt;
&lt;br /&gt;
&lt;br /&gt;
&#039;&#039;&#039;Claims will not be processed if any of the following are missing or incomplete:&#039;&#039;&#039;&lt;br /&gt;
&lt;br /&gt;
* A Completed Accident Insurance Claim Form (two pages)&lt;br /&gt;
** Both parts of the claim form must be completed including &#039;other insurance&#039; portion of the form&lt;br /&gt;
** Page 2 requires the &amp;lt;u&amp;gt;signature of the Regional Commissioner&amp;lt;/u&amp;gt; or Safety Director for the Region&lt;br /&gt;
* Copies of primary Explanation of Benefits (EOB) from the primary insurance carrier&lt;br /&gt;
* Copies of itemized expenses: insurance billing forms – UB04 or CMS1500 or detailed forms with itemized charges and diagnosis codes, these can be requested from the primary insurance carrier&lt;br /&gt;
* Copies of receipts for anything paid directly by the claimant&lt;br /&gt;
* AYSO Incident Report, filed with the Office.&lt;br /&gt;
&lt;br /&gt;
&#039;&#039;&#039;Primary Insurance:&#039;&#039;&#039; If there is no other insurance available to the registered member, the medical benefit will be processed on a primary basis subject to Usual and Customary rates, and the policy terms, conditions and exclusions.&lt;br /&gt;
&lt;br /&gt;
&#039;&#039;&#039;MAXIMUM BENEFITS PAYABLE:&#039;&#039;&#039; (subject to policy terms, conditions &amp;amp; limits)&lt;br /&gt;
&lt;br /&gt;
* $50,000 Accident Medical Maximum per Injury including Dental and Orthopedic Benefit.&lt;br /&gt;
* $15,000 for Accidental Death &amp;amp; Dismemberment.&lt;br /&gt;

All AYSO currently registered* members (players, coaches, managers, team workers, referees, officials and volunteer workers) are “Covered Persons” for accidental bodily injury while participating in the following covered activities:&lt;br /&gt;
&lt;br /&gt;
* Team practice sessions, scheduled games, tournaments, or other sponsored activities (meetings, banquets, fundraisers) provided they are under the direct supervision of an AYSO registered volunteer. &lt;br /&gt;
* Travel of covered members to and from a sponsored activity such as practice sessions, games, tournaments, or AYSO sanctioned activities, provided that players are traveling as a team and a licensed adult driver operates the vehicle. &lt;br /&gt;
&lt;br /&gt;
Registration requirements will be verified before any benefits are paid.&lt;br /&gt;

Once the claim has been submitted to Player&#039;s Health, an email confirmation will be sent to confirm the submission of the claim to Administrative Concepts, Inc., AYSO&#039;s TPA (third party administrator), generally within 5 business days.&lt;br /&gt;
&lt;br /&gt;
Once the processing agent has received the claim, a second email will be sent from Administrative Concepts, Inc., generally withing 10 business days (check your junk/spam folders).  All correspondence regarding the claim should be sent to the claims Administrator from this point forward. &lt;div&gt;AED are used to help people experiencing sudden cardiac arrest by analyzing the heart&#039;s rhythm and delivering an electrical shock to restore a normal rhythm.  The state of New York has mandated the presence of AED&#039;s and CPR/AED trained AYSO volunteers at AYSO functions.  This program is &#039;&#039;&#039;limited to Regions and Areas in the state of New York at this time.&#039;&#039;&#039;&lt;br /&gt;

===Plan Requirements===&lt;br /&gt;
New York state law, effective May 22, 2024, requires youth sports organizations, including AYSO, to provide an AED and a trained provider at every AYSO function including games, practices, camps and clinics.  A Public Access Defibrillation (PAD) Program must have the following elements:&lt;br /&gt;
&lt;br /&gt;
#&amp;lt;u&amp;gt;Medical Direction&amp;lt;/u&amp;gt; - New York State licensed physician or New York State based hospital knowledgeable and experienced in emergency cardiac care to serve as Emergency Health Care Provider (EHCP) to participate in a written collaborative agreement.&lt;br /&gt;
#&amp;lt;u&amp;gt;Appropriate Number of AED Devices&amp;lt;/u&amp;gt; - Select an AED that is in compliance with the Article 30, section 3000‐B (1)(A) which must be programmed to the current Emergency Cardiovascular Care (ECC) Guidelines, and capable of defibrillating both adult and pediatric patients.&lt;br /&gt;
#&amp;lt;u&amp;gt;Accredited Training in the Use of AED&amp;lt;/u&amp;gt; - Select and use an approved PAD training course for AED users.  The State of New York has specified training must be provided by one of the following agencies:&lt;br /&gt;
##American Heart Association &lt;br /&gt;
##American Red Cross &lt;br /&gt;
##American Lifeguard Association&lt;br /&gt;
##American Safety and Health Institute&lt;br /&gt;
##EMS Safety Institute&lt;br /&gt;
##National Safety Council &lt;br /&gt;
#&amp;lt;u&amp;gt;911 Emergency Medical Services&amp;lt;/u&amp;gt; - Provide written notice to the 911 center in your county of the availability of AED service at the  organization’s location.&lt;br /&gt;
#&amp;lt;u&amp;gt;Post Event Reporting&amp;lt;/u&amp;gt; - Every use of an automated external defibrillator on a patient shall be immediately reported to the appropriate local emergency medical services system, emergency communications center or emergency vehicle dispatch center as appropriate and promptly reported to the emergency health care provider.&lt;br /&gt;
# &amp;lt;u&amp;gt;PAD Program Plan&amp;lt;/u&amp;gt; - Regions or Areas registering players, must provide a copy of your written PAD Program Plan and signed Collaborative Agreement to your Area Director or Section Director, respectively.&lt;br /&gt;

To prevent catastrophic outcomes, AED&#039;s must be accessible and properly maintained, these requirements include:&lt;br /&gt;
&lt;br /&gt;
#&amp;lt;u&amp;gt;Testing and Testing Log&amp;lt;/u&amp;gt; - each AED must be tested monthly in accordance with the manufacturer&#039;s guidelines.  Testing date, time, and name of person conducting the test must be logged.  An image of the log entry should be photographed for the date and time stamp and emailed to your designated PAD Coordinator (CVPA or Safety Director, as suggestions).&lt;br /&gt;
# &amp;lt;u&amp;gt;Replacement of Consumables&amp;lt;/u&amp;gt; - AED Consumables primarily include the battery and pads.  Replacement is required after use or as indicated during testing.&lt;br /&gt;
#&amp;lt;u&amp;gt;Post Use Protocol&amp;lt;/u&amp;gt; - After use, your AED must be tested and all consumables replaced.&lt;br /&gt;

===Equipment Storage &amp;amp; Insurance===&lt;br /&gt;
It is &#039;&#039;&#039;NOT&#039;&#039;&#039; recommended for AYSO&#039;s AED Equipment to be stored in a public place.  Cabinets with AED Equipment are subject to damage, theft, vandalism and the elements.  &amp;lt;u&amp;gt;Loss or damage to the AED is the responsibility of the Region&amp;lt;/u&amp;gt;.  It is recommended, to safeguard against theft or damage, that equipment insurance be purchased.  Region equipment IS NOT covered under the AYSO Office&#039;s equipment policy.  Insurance is available at a discount through our insurance partner. The agreement is a comprehensive approach to a PAD Program and includes equipment, services, and if elected, training of eight volunteers per AED purchased through a Purchase Plan.&lt;br /&gt;

&#039;&#039;&#039;The pricing listed below is per AED and includes services as outlined in the Kit details for the number of years equivalent to the payment term.&#039;&#039;&#039; Also included in the tables is the estimated cost of equipment, training and consumables over the 8 year warranty period.  Training is calculated based on 8 volunteers x $75 per participant, as offered by Cardio Partners where not included in the term, and each of the required 2-year renewal periods, battery and pad replacement assumes a single replacement in year 4 of ownership and does not consider the replacement of consumables in the event of use.&lt;br /&gt;

#&#039;&#039;&#039;AED INSTALLATION SERVICES:&#039;&#039;&#039;  For each AED purchased under an Equipment Purchase Plan, the Area or Region will be provided delivery and installation of the AEDs to the designated locations by Cardio Partners.  Cardio Partners will also test the AEDs to ensure they are rescue ready as part of the installation. &lt;br /&gt;
#* Initial and ongoing analysis which meet industry standards, of where AED units are required, including determination of the number of units needed to meet applicable laws, guidelines, and regulations.&lt;br /&gt;
#*Completing agency notification pertaining to AED installation, where required.&lt;br /&gt;
#*Access to compliance management online portal&lt;br /&gt;
#*Review known and/or anticipated county-level, city-level and other local AED requirements (i.e., proposed bills) to provide the Area or Region an assessment report that meets industry standards.&lt;br /&gt;
#* Assistance with registration of the Area or Region&#039;s AEDs (when required by law).&lt;br /&gt;
#&#039;&#039;&#039;AED COMPLIANCE MANAGEMENT SERVICES:&#039;&#039;&#039;  For each AED purchased under an Equipment Purchase Plan, the Area or Region shall be provided AED compliance management services as follows: &lt;br /&gt;
#*Cardio Partners will set up each AED and will maintain the AED and CPR training records&lt;br /&gt;
#*Access to the AED Law Center which shall include direct access to a database of local and state regulations regarding AEDs and/or CPR training&lt;br /&gt;
#*Monthly AED inspection reminders, expiration date tracking of pads, batteries, and training, post event support, and where required by law or regulation, medical direction and EMS notification.&lt;br /&gt;
#*In the event an AED has been used in a response to a cardiac event during the term of the Schedule (“Cardiac Event”), Cardio Partners will provide the following support:&lt;br /&gt;
#*#Contact the the Area or Region location within twenty-four (24) business hours or the following business day of being notified of the Cardiac Event to review the post event process with the Area or Region.&lt;br /&gt;
#*#Notify local EMS of the Cardiac Event within the time period required by applicable law and/or regulation; and provide the Area or Region with proof of notification which shall be viewable in the web portal.&lt;br /&gt;
#*#Send a loaner AED to the location via overnight shipping to arrive on the next business day.&lt;br /&gt;
#*#Replace the AED pads and CPR Response Kit.&lt;br /&gt;
#*# Provide a written summary report of the Cardiac Event data and data analysis.&lt;br /&gt;
#&#039;&#039;&#039;CUSTOMER SERVICE SUPPORT&#039;&#039;&#039;: An Account Manager will be assigned to the Area or Region and will be the single point of contact for all matters related to  the Agreement.  Cardio Partners will respond to questions and service requests within twenty-four business (24) hours.&lt;br /&gt;
#&#039;&#039;&#039;ON-GOING AED MAINTENANCE SERVICES (“MAINTENANCE PROGRAM”)&#039;&#039;&#039;: including on-site inspections, consumables replenishment and compliance services.&lt;br /&gt;
